What is this dispenser called and where can i find it?

They are called foaming blocks. Yours are used for triple foam from what I can tell. Every manufacturer makes their own version. Just call the manufacturer of your machine or tunnel to get them. Aftermarket companies like TSS and Vaughn also sell them.
