I had a couple leakers and rebuilt them yesterday. The leakers were exactly as the recall photos that Diamondwash posted. The one backup ring wasn't complete- ends weren't meeting correctly. Made the O-ring in that spot bulge out and I assume cause the leaks out the grease fittings.
