Cracked Cat 310 Pump

Has anyone ever seen anything like this with a Cat 310 Pump. In 42 years of being in the car wash business I've never seen anything like this. I told my customer lets let it run until it uses up all of it's nine lives. This pump is about 4 years old.

We have a cat 3535 in one of our ER that came with the car wash, broken back cover, looks like a rod broke and took out the cover
 
A few years ago I had a 5CP pump and the rod broke. It cracked the back cover and all the oil eaked out. When I discovered it the pump was still running and producing about 800 psi!

My first guess would be that the belts were changed by screwdriver. I've been guilty of that myself a few times but luckily no case fractures. Second guess is hydrolock. Unloader malfunction, a d¡ckhead customer pulling the trigger rapidly and repeatedly, or a sudden tip plug up.
Third and final guess is an uneven mount surface combined with over tightening of the fasteners.
I'd sure love to know the answer to that one!
