You probably lost a bearing if it locked up enough to turn the shaft in the bearing. The only right fix is a machine shop repair (and new bearings), or new wheel (and new bearings). A machine shop could turn it down flat and cap it, depending on how worn down it is.

Pretty sure it just means you had a bad swipe of a preloaded card. One way a card reader can fail is it will generate fake data and think you are swiping a card constantly. The button will give you a button input error instead. Like MEP said, try swapping your card reader.
