Thank you for your response, we had a local carwash mechanic come in and found out it was actually one of the bearings that had busted. Experience goes a long way, we are grateful to have found some local help! Conveyor is fixed and running well again.

Hello! We have an old Belanger dura trans XD conveyor, what could be causing the chain to slip off the sprocket? - chain tension from the pressurized air cylinders is good - the entire take-up assembly seems to be turning/tilting to one-side before the slip occurs - bearings seem to be okay...
